Click on the image(s) below to download them. You can personalize these images with your practice information, by following the instructions below and using Microsoft Paint.

1. Open the file using Microsoft Paint.
2. Right click the image you would like to add your practice name, phone number, website.
3. Within the drop down, click edit to open.
4. At the top of the bar, click the “A” for text. Move your cursor to where you would like to type.
5. Add your practice name, phone number, website in font “Arial” and size 48 centered.
6. Save file.

This customizable template for Pediatricians and Family Practitioners can be utilized to send to families either via mail or email encouraging and reminding them to return to the office for well-child visits and vaccinations prior to returning to school. Be sure to include this message on your organization’s letterhead.
