Responsibilities
• Works with multiple sites/agencies to identify, assess, and improve nutrition and physical activity in community worksites;
• Serves as a point of contact with the NYSDOH in regard to the Creating Healthy Schools and Communities grant;
• Prepares annual budgets and work plans to submit to the NYSDOH for approval;
• Works with Seneca County Finance Department and the Public Health Director to effectively manage the grant budget;
• Coordinates training and technical assistance programs for large numbers of diverse professional staff;
• Coordinates meetings with stakeholders; including the creation and distribution of meeting notices and agendas;
• Develops short, medium and long-term plans and goals for implementation of the CHSC grant;
• Develops or identifies materials and resources, and training needs;
• Prepares and submits timely and accurate detailed reports as required by the NYSDOH and program partners;
• Actively participates on the Seneca County Health Solutions Committee;
• Facilitates the incorporation of grant deliverables to coordinate with County CHIP (Community Health Improvement Plan) priorities and activities;
• Provides quarterly reports on progress towards objectives and performance measures from the CHSC work plan;
• Conducts on-going evaluation of the CHSC program;
• Develops promotional materials such as success stories and press releases about program activities;
• Gathers and analyzes data and presents findings in a logical, concise manner;
• Other duties as assigned including (but not limited to) participation in departmental drills and clinics as part of Public Health Emergency Preparedness and Response activities.
Minimum Qualification
A. Possession of a Bachelor’s degree in Nursing, Public Health, Public Health Education, Nutrition, Community Health, Urban Planning, Community Development, Grants Management, Human Services, Education or a closely related field AND
B. Experience involving the administration and development of grants, or significant closely related experience.
Special Requirement
VACANCY: At present there is one (1) vacancy for this position in the Seneca County Public Health Department.
RESIDENCY: Candidates must have been legal residents of Seneca County or one of the six (6) contiguous counties for one (1) month preceding the date of application. Preference in appointment may be given to successful candidates who have been legal residents of the municipality in which appointment is to be made for at least one (1) month prior to the date of certification and are residents of such municipality at the time of appointment.
Possession of a valid New York State Driver license is required at time of appointment and must be maintained on a continuous basis throughout the term of employment.
Salary: $56,475-$74,907
